The HPA-HPO Axis Connection
Your body has two major hormonal control systems: the HPA axis (hypothalamic-pituitary-adrenal, governing stress response) and the HPO axis (hypothalamic-pituitary-ovarian, governing reproduction). These systems share upstream hormonal pathways and directly communicate. Under chronic stress, the HPA axis suppresses the HPO axis, essentially telling your reproductive system that this is not a safe time to reproduce.
How Cortisol Disrupts Your Cycle
Elevated cortisol suppresses GnRH (gonadotropin-releasing hormone), which in turn reduces FSH and LH production. The result: delayed or absent ovulation, lengthened cycles, shortened luteal phases (which can impair implantation if trying to conceive), and worsened PMS as progesterone production is reduced. In extreme stress, such as intensive athletic training, severe calorie restriction, or major life trauma, periods can stop entirely (hypothalamic amenorrhoea).
The Cortisol-Progesterone Steal
Both cortisol and progesterone are synthesised from pregnenolone. Under chronic stress, pregnenolone is preferentially shunted toward cortisol production, leaving less available for progesterone synthesis. This "cortisol steal" is a real biochemical phenomenon that contributes to luteal phase deficiency, pre-period anxiety, and difficulty conceiving. Addressing the stress is not a soft recommendation, it is medical management.

Mindfulness-based stress reduction (MBSR) has demonstrated measurable reductions in cortisol and improvements in menstrual cycle regularity in clinical studies.
